Information, Services, and Guidelines for Patients

Access Your Health Care Information Online

As part of our service, our current patients can access their health care information, including viewing Lab/Xray results, and make and confirm appointments by logging on to www.nextMD.com. We will arrange it so you can access your records online through this site. Please ask our staff for more information about this service.

 

 

Our Patient Partnership Plan

We intend to provide you with the care and service you expect and deserve. Achieving your best possible health requires a "partnership" between you and your doctor. As our "partner" in health, we ask you to help us in the following ways:

  • Schedule visits with My Doctor for Routine Physical Exams and Other Recommended Health Screenings
  • Keep Follow-Up Appointments and Reschedule Missed Appointments
  • Call the Office When I Do Not Hear the Results of Labs and Other Tests
  • Inform My Doctor If I Decide Not to Follow His or Her Recommended Treatment Plan

 You have the right to be informed about your health care. We invite you, at any time, to ask questions, report symptoms, or discuss any concerns you may have.

 

 

Appointments

In order to better serve you, please come in 15 minutes before your scheduled appointment time to ensure you see the physician in a timely manner. This will maximize your encounter time with the physician.  This allows time for the staff to confirm your insurance and check you in. The medical assistant will room you by checking your vital signs and problem list.  If you are running late for a scheduled appointment, we have the right to reschedule. If you call ahead that you are running late, we will make every effort to accomodate you but you may have to wait. To minimize your wait time, please notify the staff of any recent Xray or Laboratory results or if you need a referral to see a specialist. If you cannot keep your scheduled appointment, please inform us in a timely manner so the physician can utilize the time slots for other patients with urgent problems.

Walk-ins are not encouraged, but the medical assistant will evaluate your condition and confer with the physician. Please be patient with the staff while they wait for a response from your primary care physician.

 

Urgent Visits

Please call the office as early as possible to if you need a same day urgent visit. This allows the physician to adjust the schedule accordingly depending on the nature of your complaints. If you do not receive a return call within 24 hours, please call us back. Please inform the staff of your first call regarding the urgency of your complaints. The medical assistant will triage your call and speak with the physician to determine when you need to be seen.

Please call 911 if your call is life threatening. The paramedics will bring you to the nearest hospital. The emergency room doctor will inform your primary care physician of your condition.

 

Insurance and Copayments

Please bring your insurance card with you to every visit and let the staff know of any changes. Copayments if required are due at the time of check in. Patients with outstanding balances will be required to make a payment before being seen. Our staff may re-schedule your visit if payment has not been received.

 

Referrals

If you need a referral to see a specialist, please call the clinic for the authorization.  Please allow the clinic ample time to verify your insurance eligibility and check the preferred provider network of specialists.  It is the patient's responsibility to pick up the referral. We do not fax them. No referrals will be given after the patient has been seen by  the specialist. Some specialists require two weeks notice for approval from your health plan depending on your benefit and required documentation to support the referral. Please be patient with the referral process and remember your insurance requires us to get prior authorization for any referral to a specialist.

 

 

Xrays and Lab Requests

Orthopedics and Podiatrists require recent xrays and films to be brought at the time of your appointment. Check with the radiology facility regarding requirements for pick up of films or discs. If you need a lab or xray request form, please give us 24 hours notice.

 

Prescription/Refills

For any 30 day prescriptions to the local pharmacy, please contact them directly. Please contact the pharmacy 2 to 3 days before you run out of medications. Please have them fax the requests to our office. Please allow the physician 24 to 48 hours notice to fill  your prescriptions especially on the weekends and holidays. For mail order refills, please makes sure you bring the refill forms provided by the mail order pharmacies to expedite your requests. We do not call the mail order pharmacies for any refills.

 

Routine Phone Calls

When you leave a message for a call back, please remember to give your name and phone number where you can be reached, and specific reasons for the call. There will be 24 hours call back especially during a busy day. Please remember, the staff maybe busy with the other calls so please keep trying if you can not reach us on the first call.

 

Behavior

Anyone using profanity will not be seen!!! Please be patient and kind to the staff and remember common courtesy will be greatly appreciated especially during a hectic day.
